The Lizard Formula
Melati Kusuma came to get a scholarship and was enrolled in Empire State University where she got the internship with Dr. Curt Connors. He quickly decuded though that Kusuma was interested in his formula as a means to restore her legs that she lost in a car crash. Dr. Connors promised that they would together work on making the formula safe first before she could use it. He would ask her to swear that she would not do anything reckless in the meantime with Melati agreeing to do so. This was the case for months until Connors let Melati work alone at one of his auxiliary laboratories. It was then that she decided to steal the formula and test it on herself. It came to work in that it restored her legs and gave her reptilian traits but she retained her mind thus for her it was deemed a success. At first, Connors was angry with her but came to understand her desire to have her legs restored to her. It was determined that her solution for the formula would only work on her DNA and thus it could not be applied as a cure for the wider public. Ultimately, Connors had Melati swear to use her new powers responsibly where she enrolled in the Initiative program as Komodo. (Avengers: The Initiative Special v1 #1)

- The Lizard Formula was created by Stan Lee and Steve Ditko where it made its first appearance in Amazing Spider-Man v1 #6 (August, 1963).

Television
- In Spider-Man, an unknown serum appeared in the setting of the 1967 animated television series in the episode "Where Crawls the Lizard". Dr. Connors was looking to develop a cure for 'swamp fever' where he conducted his research in the Florida Everglades. The serum though came to accidentally transform him into a reptilian mutation known as the Lizard until Spider-Man eventually helped cure him.
- In Spider-Man: The New Animated Series, the unknown formula appeared in the setting of the animated television series in the episode "Law of the Jungle". Dr. Curtis Connors was a professor at Empire State University; who worked alongside Peter Parker in researching Lizard DNA in hopes of developing a serum that; would grow back amputated limps. In doing so, he tried his experimental serum on himself and ended up mutating himself; into a horrid lizard-like creature. He retained his knowledge and sought vengeance against Oscorp; due to their involvement in the loss of his limbs.
Films
- In The Amazing Spider-Man, the formula appeared in the setting of the live-action film. It was the work of Dr. Curt Connors who was a geneticist at Oscorp where he specialised in cross-species genetics. Longing for his missing arm, he looked into research that was made by Dr. Richard Parker in cross-species work. However, Connors work was stifled due to a missing piece which was the decay rate algorithm. This was later provided to him by Peter Parker the son of Richard Parker. Completing his formula, he showcased it to Oscorp executive Rajit Ratha who pushed for human testing. However, Connors refused to do so leading to him being fired and Ratha intending to test the formula at a Veteran's Administration hospital. In desperation, Connors stole the formula and tested it on himself. After falling unconscious, he awoke to find his arm had regenerated but then he mutated into a giant humanoid reptilian monster. With his mind warped, Connors as the Lizard looked to release a biochemical agent from Oscorp tower to turn all humans in New York into reptilian hybrids like himself, eliminating the weaknesses he believes plague humanity. He came to be opposed by the masked superhero Spider-Man who ultimately thwarted the Lizard's plans with Gwen Stacy creating an antidote that turned the monster back into Curt Connors.
- In Spider-Man: No Way Home, an antidote to the unknown serum appeared in the setting of the live-action film set in the Marvel Cinematic Universe. The Lizard from Amazing Spider-Man came to be accidentally teleported across the Multiverse to another Earth where he came into conflict with that world's Spider-Man. Peter Parker came to work on developing a cure for him but initially failed until he received help from the other reality counterparts of himself. One of whom managed to inject the antidote into the Lizard thus turning him back into Dr. Connors.
Video games
- In Spider-Man 3, the unknown formula appeared in the setting of the video game tie-in to the Spider-Man trilogy. Dr. Connors decided to test out a formula he developed that would in theory regrow his lost limb. He created a video diary where he told the viewer to give a letter to his wife and son if anything should go wrong; and something did go wrong. The video continued on to show Connors use the formula, he commented that it stung much more than he expected, and he threw the formula at the camera in a bout of sudden anger, ending the video. Dr. Connors was then transformed into the Lizard. A deranged creature that still maintained some of Connors' human intelligence. When word had gotten out that giant lizards were running amuck in the city, Jameson sent in Peter Parker to grab photos. When he did Peter, as Spider-Man, found himself confronting hordes of lizard men; products of the actual Lizard's experiments. Spider-Man would find himself thrust below ground, fighting off lizards within the city's sewer systems where the experiments were taking place. It was not long before Spider-Man confronted Connors himself, deep in the sewers. The battle ended with a Spidey victory, however Peter wasn't able to stop Dr. Connors from escaping
